Professional licensure disclosure
Postsecondary institutions participating in federal student aid programs are required to make certain disclosures to enrolled and prospective students, parents, employees and the public. The U.S. Code of Federal Regulations ( 34 §C.F.R. 668.43) requires disclosures by institutions regarding educational requirements for programs leading to professional licensure or certification regardless of modality. The institution must make readily available to enrolled and prospective students information about whether programs leading to professional licensure or certification meet educational requirements.

Determination of Student Location Policy
In accordance with U.S. Department of Education regulations (34 CFR § 668.43(c)), Carl Sandburg College officially determines a student's physical location using the permanent address submitted on their initial application for admission.
Mandatory Relocation Notification: If a student permanently relocates to a different state at any point during their enrollment in a healthcare program, they are required to update their address with the Registrar's Office within 14 calendar days.
Please be advised that relocating to a state or territory listed under Table B (Does Not Meet / Has Not Been Determined) may negatively impact your ability to fulfill clinical rotations, sit for licensing exams, or secure employment in that jurisdiction. If an address update places a student in a Table B jurisdiction, Carl Sandburg College will issue a direct, individual written disclosure notice via student email within 14 calendar days of receiving the change of location.
